The University of Ibadan has reopened the University Health Service (UHS) requisite form for returning students.

The development was disclosed in a memorandum signed by the Dean of Students, Professor Demola Lewis, on Friday, March 6, 2026.

“We are sending out a mop-up UHS requisite form strictly for returning students who missed filling the first one,” the statement reads.

According to the memorandum, the mop-up form will remain open until midnight on March 6, 2026.

Recall that the university earlier opened the form as part of the requirements for securing accommodation in the halls of residence for the 2025/2026 academic session.

Affected students are advised to complete the mop-up form using the link provided and upload a scanned copy of their Jaja Card during the application process.
